Community Care Service Program (CCSP)

The Community Care Service Program (CCSP)
is a waiver program under Medicaid which provides
community-based services to the elderly and those
with physical disabilities. This program allows
members to receive personal care services
in their homes, an alternative to a nursing
home or assisted living.

Services Provided

• Skilled Nursing
• Wound Care
• Light Housekeeping
• Meal Preparation
• Medication Management
• Home-delivered meals

Eligibility

• Must have physical disability (Alzheimer’s and Dementia are valid disabilities)
• Physician approval stating that the individual’s level of care meets the requirements for a nursing home
• Medicaid eligible
• Must agree to received personal services in their home
