Independent Care Waiver Program (ICWP)

The Independent Care Waiver Program (ICWP) is
specially created for Medicaid eligible recipients
with physical disabilities to receive personal
home health care services; this is an alternative
to a healthcare facility (Nursing Home or Hospital).
This waiver program is also extended to persons
with traumatic brain injuries (TBI).

Who Qualifies?

• Individuals who are between ages 21 and 64
• Medicaid approved
• Individuals with severe physical disability
• Must be able to conduct their own services

*Members with TBI are exempt from this standard

• Case Management
• Personal Care
• Skilled Nursing
• Transportation
