The Georgia Pediatric Program (GAPP) is
a State funded program that was specially
created for children ages 0-21 years, who
require long-term or short-term in-home
medical nursing care.

• Skilled Monitoring
• Home Infusion
• Post-Surgical Care
• Wound Care
• Personal Care

Does your child qualify?

• Member must be age 0-21 years
• Require medical nursing care
• Require personal care
• Medically fragile
• Must currently be receiving physician-ordered services
