waiver program that provides primary
care as well as case management to
people who are physically disabled, frail,
and elderly, also persons with chronic
medical conditions.

• Personal Homecare
• Respite Care
• Companion Care
• Wound care
• Meal Prep
• Light Housekeeping
• Skilled Nursing

Eligibility

• Individuals must be Medicaid eligible
• Medicaid approved
• Level of Care must meet nursing home standards
• Medical conditions (diabetes, pulmonary disease, Alzheimer’s)
